Messages


We understand that at times you have questions or concerns that you would like to address with your provider. These are best handled if you call in the morning, so we can get you an appointment with your provider.  Please understand that a new prescription and antibiotics will not be prescribed without an office visit.  The provider will need to see you to best determine which medication would treat your symptoms.


Front staff can also send a message to your provider on your behalf. Providers view their messages throughout the day while attending to patients that are here to be seen, and are address accordingly.

Referrals
At times, your physician will discuss and recommend that you go to a specialist or facility for specific consultation or testing.  In most instances, you will require a referral in order to be seen.  Please check with your insurance plan to see if there any specific requirements that they have in regards to referrals.  Referrals may be given to you after evaluation at office visit and/or test results.

Prescriptions

Please initiate your refill requests with your pharmacy.  Refill requests are processed within one-two business days.



Medical Records


Medical records are released when we are provided with a signed release form from the patient or legal guardian.  We are responsible for the protection of these records and must have proper documentation for release.  Exceptions from some third party entities override patient consent.


There is a $20.00 medical record request fee.  Payments accepted are cash or check only.  Make out check to: SHARECARE.  Return form and payment to our office for processing. 

Lab and Diagnostic Results
Please allow up to two weeks for lab and/or diagnostic results. 


All normal (only) lab results will be available in Mychart after provider has reviewed. Sensitive results will not be available.


Your provider will contact you to review results that will require immediate attention. 


Please call the office if you wish to receive a paper copy of results. 


Staff is not able to release any results over the phone without provider's consent.
